PRESSURE ULCER PREVALENCE
Good Samaritan Medical Center’s healthcare team is dedicated to providing evidenced based assessments and clinical care to our patients for the prevention and treatment of pressure ulcers. Our dedication and focus in this effort includes constant education on nursing assessment, documentation and prevention protocols. Risk assessments are completed on admission and every shift by registered nurses. Our Interdisciplinary hospital-wide Skin Care Team in comprised of Certified Wound Ostomy Nurses (CWON), Clinical Leaders, staff RNs that are unit based dedicated Skin Champions and other disciplines as necessary. Prevalence studies are completed quarterly.

FALLS
Good Samaritan Medical Center’s falls prevention program is centered around on-going risk assessments, Good Samaritan Medical Center’s falls prevention program is centered around on-going risk assessments, beginning on admission, evidenced based practice, and staff awareness and education. The Falls Team is interdisciplinary, comprised of clinical education, transport team, physical therapist, pharmacists, nursing leadership, registered nurses, and certified nursing assistants. The team meets monthly and continues to explore and implement best practices including shift change safety huddles, improved communication and between RNs and CNAs, improved safe hand over between RNs shift to shift and post fall assessments and review.
